realizando un sitio web con una base de datos resulta que todo bien en la pc donde realize el codigo me hace registros los guarda y los consulta

pero a la hora de ponerlo en el servidor me muestra la pagina inicial y cuando quiero agregar un registro nuevo me aparece este codigo:
